Information We Collect

We collect personal information from you, including information about your:

• name
• contact information
• date of birth
• address
• email
• NHI (if known)
• contact number.

We use this data at booking so that we can add you to our medical system. If you do not want to send this via the questionnaire feel free to send via:

A medical questionnaire is sent via timely and you have the option to complete this electronically and it will be added to your medical record or if you prefer we can complete this in clinic. 

Besides our staff, we share medical information with your consent, with:

• your primary care doctor or
• other health care providers as discussed with you such as needed for referrals, lab requests or radiology requests. 

Providing some information is optional. If you choose not to enter appropriate documentation for enrolments, we’ll be unable to complete your enrolment and will require you provide the information during your consultation.

We keep your information safe by storing files on our secure medical system.

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.

Software

Our bookings and questionnaire go through timely and payments are process via timelypay and stripes.

Our medical system is Indici. Your medical records are stored in this system.

Our telehealth consults are conducted via Zoom or if you prefer can be conducted via telephone.

What is HealthOne?

HealthOne is a South Island based secure electronic record that allows registered healthcare providers directly involved in your healthcare, to quickly access information such as your test results, allergies, medications, GP summaries and hospital information. HealthOne adheres to the principles of the Privacy Act 2020 as well as the Rules set out in the Health Information Privacy Code 2020.  Access is only possible via an approved highly secure healthcare information network which is regularly audited and tested.  Privacy auditing is used to check that only those directly involved in your care are accessing your information.
